It was written by by English Roman Catholic composer, church-musician and social-justice activist Bernadette Farrell (b 1957). The first publication was in 1986 by the American-based OCP Publications. Since then it has been included in both US, UK and Australian hymnals. It is widely used as a congregational hymn, but it doesn't appear to have been recorded by any other singers.
Also, although it is written as a full hymn, some people have noted that the chorus alone can be used as a Gospel Acclamation during Lent in Roman Catholic church services.
It is set to an unnamed tune, also by Farrell. This has a 4/4 time signature. In the key of Em, the first few notes of the chorus melody are E ED E B A GA B E with accompanying chords Em / Bm / Em / Am / D / Em.

Lyrics
The lyrics are copyright so cannot be reproduced here. But they are currently available on this website. A rough outline is:Praise to you, O Christ, our Saviour,
Word of the Father, calling us to life;
Son of God who leads us to freedom:
Glory to you, Lord Jesus Christ!
1 You are the Word who calls us out of darkness ...
Gory to you, Lord Jesus Christ!
2 You are the one whom prophets hoped and longed for ...
3 You are the Word who calls us to be servants ...
4 You are the Word who binds us and unites us ...
