This is a hymn about Jesus in the Eucharist.
It was written by American Roman Catholic musician, vocalist and music-director / worship-director, Jim Cowan (b 1952), and first published by International Liturgy Publications in 1987. Sheet music is still available from them in various formats - link below.
It is set to an unnamed tune, also by Cowan. This has a 4/4 time signature. In the key of C, the first few notes of the melody are C D E G G__ / G A G G__ with accompanying chords C2 C F C.

Lyrics
The lyrics are copyright so cannot be reproduced here. But they are currently available on this website. A rough outline is:Chorus:
This is Jesus, Emmanuel.
God is with us in this sacrament,
come receive him, adore him.
This is Jesus, our Lord.
1. Bright Star of David radiance of God ...
2. Drink of living water flowing from the Lord: ...
3. Priest of highest honour, King and sovereign Lord ...
Unblemished Lamb of sacrifice, your very life outpoured.
Royal Paschal victim, true begotten Son,
offered as our ransom by the Holy One.
