It was written by English Roman Catholic composer, church-musician and social-justice activist (ref) Bernadette Farrell (b 1957). in 1990.
It was first published by USA-based OCP Publications, and sheet music for it is still in-print - links are given below.
The hymn is set to an unnamed tune, also by Farrell. This has a 4/4 time signature. In the key of A, the first few notes are EAG A_ BGF E E with accompanying chords A / D / E / A.
