It was written by English Roman Catholic organist, music director and composer Paul Inwoood (b 1947), and first published through the St Thomas Moore group by USA-based OCP Publication, in 1987. It doesn't appear to have been widely published since then, though it was included in the end edition of the American Journeysongs, and sheet music is still available from OCP (link below).
