No currently in-print sources of sheet music have been located:  a second-hand copy of one of these books is the best option.

It is set to an unnamed tune, also by Bryant. This has a 4/4 time signature. In the key of Eb, the first few notes of the melody are GA GA GA B_ AA GG AG F_ with accompanying chords Eb / Gm7 / Ab/ Eb / Fm7.

Lyrics

The lyrics are copyright so cannot be reproduced here. But they are currently available on this website. A rough outline is:
Jesus, take me as I am.  I can come no other way.
Take me deeper into You.  Make my flesh life melt away.
Make me like a precious stone,  crystal clear and finely honed,
Life of Jesus shining through giving glory back to You.

KEEP IN CASE ORIGINAL IS REMOVED, BUT DO NOT DISPLAY Jesus, take me as I am.
I can come no other way.
Take me deeper into You.
Make my flesh life melt away.
Make me like a precious stone,
Crystal clear and finely honed,
Life of Jesus shining through
Giving glory back to You.
