The song was written by English Roman Catholic liturgical musician and publisher Kevin Mayhew (1942-2021), and first published in 20th Century Folk Hymnal, Volume 3 - Kevin Mayhew Ltd, England in 1976.
It was included in a radio service on BGC Radio 4 in 1979 (source), and later included in Hymns Old and New - With Supplement - but not the 2009 Catholic edition. No more publications have been found, although Youtube shows the song is used in parts of Asia / India.
It is set to an unnamed tune, also by Mayhew, which appears to borrow from Israeli folk-music idiom. The tune has a 4/4 time signature. In the key of Em, the first few notes of the melody are E E D BD E E E with accompanying chords Em / D / Em.

Lyrics
The lyrics are copyright so cannot be reproduced here. But they are currently available on this website. A rough outline is:Chorus:
On we go to Jerusalem
The city of the house of God.
Pilgrim people, we seek the Lord:
The Lord of the house of Rock.
1. Age on age proclaim His praises,
Each declares Him Lord of all.
Countless millions sing His glory,
Answering His pilgrim call.
2. Peace He promised to our fathers
In the ancient desert land
Peace He offers in abundance
To His faithful pilgrim band.
3. All we have is offered to Him
As we come to sing His praise.
Blessed is the Lord of heaven,
Mighty God of all our days.
